Kindly download the other detailed guidelines apart from the following instructions from the link "DOWNLOAD GUIDELINES" given at the bottom of this page, before you click on proceed button to fill up the form.

The name of the applicant, father's name / mother's name, date of birth etc. should be spelt correctly in the form as it appears in the certificates.

You must upload recent color passport sized photo of child and each parent in JPG / JPEG file (Less than 20 kb) .

It is mandatory to fill all fields which are marked ( * ) in red. If any field is not applicable to you, please write NA (Not Applicable).

An incomplete registration form will automatically be rejected.

After completing the form, click on 'Submit' button at the bottom of the page. You will receive an sms on your registered mobile no. with USER ID & PASSWORD for further correspondence.

The USER ID & PASSWORD should be preserved by the parent to complete payment process and to take a print out of the Form through Print Form  option and Payment Receipt .

NOTE: Please note, registration of your ward will be completed only when the hard copy of the printed form is submitted in the school along with required documents by one of the parent on specified dates mentioned in the guidelines. Carry the Payment Receipt at the time of submission of form in school.
